Verification of Participation
Thank you for participating in the For Foster Dads Series. The whole goal has been to encourage you as men, husbands and dads - wherever you are on your fostering journey! It’s been a blessing to journey along with you.
There’s three simple steps to request verification for training hours…
Fill out the participation verification form.
Each module has been given a certain time-stamp based on the amount of content contained in that module. It’s up to you to determine which modules you did and did not participate in.
This is based on the honor system.

Download and save.
The form is “self-fillable”, meaning you can fill in the blanks digitally or print it out and manually write in the information. Either way, you’ll want to complete it and save it in order to turn it in to your agency.
Submit to your agency.
Turn your verification of participation letter into your licensing agency to request training hour credits. Note: We are not able to guarantee that your agency will approve your request. The only thing we are able to do is provide the documentation proving your involvement in the series based on the information you provided.
